The Clinical Care Leader (CCL) is responsible for utilizing the nursing process (assessment, diagnosis, outcomes/planning, implementation and evaluation) to provide individualized nursing care in the home setting. Collaborates with other inter-professional colleagues, to plan, implement and evaluate care. Documents patient history, symptoms, medications and care provided in compliance with applicable standards/regulations. Utilizes technology to enhance quality patient care, to collaborate with inter-professional team members, effectively delegate to and supervise unlicensed personnel, assume overall nursing accountability for a patient assignment and serve in leadership roles on department/organizational tasks forces and committees. Demonstrates competency and practices within the full scope of nursing expertise/knowledge and utilizes appropriate age and population specific standards as designated in the specific care area or unit. Communication skills to collaborate, negotiate and resolve conflicts, situations, and/or issues with peers, inter-professional colleagues, patients and families. Must have strong clinical reasoning and critical thinking skills to analyze data and appropriately respond to changing situations. coordination of care, patient assessment, patient education, triage, and various other nursing interventions. Functions within the scopes and standards of nursing practice as outlined in the Nurse Practice Act and Administrative Rules in state of practice and licensure. The Sanford Professional Nursing Practice recognizes the Scope and Standards of Practice and the Code of Ethics for Nurses with Interpretive Statements as published by the American Nurses Association as the foundation of nursing care delivery and professional conduct.
Adjusts departmental priorities and resource allocations to support changing needs.;Uses active listening skills and probing techniques to understand cause of a conflict.;Seeks opportunities to serve as a mentor.;Responsible for promoting a safe environment for patients, families, staff, residents, and visitors and working collaboratively with the multi-disciplinary health care team to provide the best outcomes for patients.;Reinforces positive contributions and recommends adjustments for improving outcomes.;Promotes an organizational culture that balances individual initiative and team collaboration and sharing of successes.;Fosters strong customer relationships via delivery on commitments, open communication, and on-going feedback/improvement.;Establishes and maintains productive working relationships within and outside of own area.;Effectively delegates and supervises based on oneâ€™s state practice act and an understanding of the concepts of responsibility, authority, and accountability.;Educates and trains others on operations, ethics, and regulations within the home healthcare industry.;Applies home healthcare operating principles and practices within own position.;Applies general clinical skills to perform clinical functions or tasks.;
Bachelor's degree in nursing strongly preferred. Graduate from a nationally accredited nursing program preferred, including, but not limited to, Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), and National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA).
Minimum of two yearsâ€™ home health nursing experience preferred.
Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state. Obtains and subsequently maintains required department specific competencies and certifications. Valid driver's license and proof of insurance required. Basic Life Support (BLS) required. Certificate for Outcome and Assessment Information Set (OASIS). Certificate for OASIS Specialist - Clinical (COS-C) preferred.
